Renal Lymphoma
Renal lymphoma refers to involvement of the kidney by lymphoma, a cancer of lymphoid cells that most commonly develops in lymph nodes, bone marrow, or the spleen. Most cases of renal lymphoma occur as part of widespread lymphoma (secondary renal lymphoma), typically non-Hodgkin lymphoma, and are identified during staging or follow-up. Primary renal lymphoma—arising within the kidney itself without evidence of lymphoma elsewhere—is much less common and remains a distinct clinical entity. Renal lymphoma has characteristic imaging patterns that can be recognized on CT and MRI. Because it typically responds well to lymphoma-directed treatment (chemotherapy and immunotherapy), distinguishing it from primary kidney cancer is important, and image-guided biopsy is often used for confirmation.
What is it?
Lymphoma is a cancer that arises from lymphocytes, the white blood cells that are central to the immune system. Lymphomas are broadly divided into Hodgkin lymphoma and non-Hodgkin lymphoma (NHL), and each includes many subtypes with distinct features and treatments. Lymphoma most commonly involves lymph nodes, bone marrow, and the spleen, but it can also involve almost any other organ, including the kidneys. The kidney does not contain organized lymph nodes internally under normal circumstances, but lymphocytes can travel through the blood and infiltrate the kidney in the setting of widespread disease or, less commonly, arise within the kidney directly.
Renal lymphoma can be categorized in several ways.
Secondary renal lymphoma is much more common than primary renal lymphoma and refers to kidney involvement in a patient who has lymphoma affecting other sites as well. It is identified in a notable proportion of patients with advanced non-Hodgkin lymphoma when imaging or autopsy studies are considered, though only a smaller proportion has clinically important kidney involvement affecting function or requiring specific attention. Aggressive lymphomas such as diffuse large B-cell lymphoma are more common causes of secondary renal involvement.
Primary renal lymphoma is rare and refers to lymphoma that arises within the kidney without evidence of lymphoma elsewhere at the time of diagnosis. Its existence has historically been debated because of the lack of native lymphoid tissue within the kidney, but well-documented cases are recognized. Primary renal lymphoma is often diffuse large B-cell lymphoma but can also include other subtypes.
Post-transplant lymphoproliferative disorder (PTLD) is a related but distinct condition that can affect the kidney, particularly in kidney transplant recipients. PTLD is generally associated with immunosuppression and often with Epstein-Barr virus infection, and can present as lymphoma involving the transplanted kidney or other sites.
Renal lymphoma has several characteristic imaging patterns on CT and MRI. The most common patterns include multiple bilateral kidney masses; a single mass; direct extension into the kidney from an enlarged retroperitoneal lymph node mass; diffuse infiltration causing bilateral enlargement without discrete masses; and perirenal disease (lymphoma surrounding the kidney like a rind). Compared with primary kidney cancers, lymphomatous lesions often show less enhancement after contrast, a homogeneous appearance, and features that reflect the tendency of lymphoma to grow within and around the kidney without significantly distorting its shape. On PET/CT, lymphomatous lesions typically show elevated glucose uptake, which is helpful for staging and treatment response assessment.
Symptoms of renal lymphoma depend heavily on the type of lymphoma, the extent of kidney involvement, and other sites of disease. Many patients have no symptoms specifically from the kidney involvement, particularly when it is identified during staging of known lymphoma. When symptoms occur, they may include flank or back pain (particularly with large masses or capsular stretching), blood in the urine, palpable abdominal mass in some cases, and, when both kidneys are extensively involved or when the collecting system is obstructed, symptoms of reduced kidney function. Some patients present acutely with kidney failure due to extensive infiltration or obstruction. Systemic (“B”) symptoms of lymphoma—such as unexplained fever, drenching night sweats, and unintended weight loss—may be present, along with other symptoms specific to the sites of disease.
Diagnosis begins with imaging, often obtained during evaluation of symptoms or, more commonly, as part of staging for known lymphoma. CT with and without contrast is the primary imaging test and can identify the characteristic patterns of renal lymphoma. MRI provides similar information without ionizing radiation. Ultrasound may identify some lesions but is less useful for detailed characterization. PET/CT is central to lymphoma evaluation and can identify lymphomatous involvement in the kidney and elsewhere.
When renal lymphoma is suspected but not confirmed, image-guided biopsy of a kidney lesion is often performed to establish the diagnosis. Core needle biopsy (rather than fine-needle aspiration) is generally preferred because lymphoma diagnosis and subtyping require adequate tissue for pathologic examination, including immunohistochemistry, flow cytometry, and molecular studies. Biopsy is particularly important when renal lymphoma is the initial presentation or when the specific lymphoma subtype is not otherwise known.
For patients with known lymphoma who develop new kidney lesions during treatment or follow-up, evaluation includes consideration of new lymphomatous involvement, progression, or, less commonly, a second unrelated diagnosis (such as primary kidney cancer or metastasis from another cancer).
Staging of lymphoma involving the kidney includes standard lymphoma staging, which typically incorporates PET/CT of the chest, abdomen, and pelvis; bone marrow evaluation in selected cases; blood tests including complete blood count, lactate dehydrogenase (LDH), and comprehensive metabolic panel; and, when clinically indicated, evaluation of the central nervous system.
Additional evaluation may include testing for viral infections (such as HIV, Epstein-Barr virus, and hepatitis B and C) that are relevant to lymphoma risk and treatment planning, and evaluation of cardiac function before receiving cardiotoxic chemotherapy.
Important to Know
Management of renal lymphoma follows lymphoma-directed treatment principles based on the specific lymphoma subtype, stage, patient factors, and, when relevant, molecular characteristics. Care is best coordinated by multidisciplinary teams that include medical oncologists (particularly hematology-oncology specialists), radiation oncologists, pathologists (including hematopathologists), radiologists, nephrologists (when kidney function is affected), and other specialists as needed. Surgery is rarely used except for biopsy or specific complications.
For diffuse large B-cell lymphoma—the most common aggressive lymphoma and a frequent cause of renal involvement—standard treatment typically includes rituximab plus a combination chemotherapy regimen such as R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisone). Alternative regimens are used in specific circumstances. Central nervous system prophylaxis with intrathecal chemotherapy or systemic high-dose methotrexate is considered in patients with specific risk factors (including extensive extranodal involvement, which can include the kidneys). Radiation therapy may be used in selected patients, particularly for bulky disease or specific sites.
For indolent (slow-growing) B-cell lymphomas such as follicular lymphoma, treatment depends on the stage, symptoms, tumor burden, and patient factors. Options range from observation (in some asymptomatic patients) to chemoimmunotherapy or immunotherapy alone. Treatment principles for kidney involvement generally follow those for the underlying lymphoma subtype.
For T-cell lymphomas involving the kidney, treatment follows regimens specific to the T-cell lymphoma subtype and is often delivered at centers experienced in these less common lymphomas.
For Hodgkin lymphoma involving the kidney—which is much less common than non-Hodgkin lymphoma—treatment follows standard Hodgkin lymphoma principles, typically involving chemotherapy such as ABVD (doxorubicin, bleomycin, vinblastine, and dacarbazine) with or without radiation depending on stage and other factors.
For post-transplant lymphoproliferative disorder (PTLD), treatment is highly individualized and often begins with reduction of immunosuppression, along with rituximab in many cases and, when needed, chemotherapy. Coordination with transplant medicine is essential.
For primary renal lymphoma, treatment principles generally follow those for the specific lymphoma subtype identified. Because primary renal lymphoma is rare, care at centers experienced in the disease is often beneficial.
Care of kidney function is an important consideration throughout treatment. When kidney function is significantly affected at diagnosis (either from the lymphoma itself or from obstruction), rapid initiation of lymphoma-directed treatment can improve function in many cases. However, tumor lysis syndrome—a rapid breakdown of tumor cells that can release large amounts of electrolytes and cause acute kidney injury—is a particular risk in patients with high tumor burden and rapidly proliferating lymphomas, and preventive measures are essential. These include hydration, medications such as allopurinol or rasburicase to reduce uric acid, and careful monitoring of electrolytes and kidney function.
Renal function considerations also influence chemotherapy dosing and choice. Medications that can harm the kidneys or that require kidney elimination may need dose adjustment or alternatives. Careful attention to hydration, avoidance of unnecessary nephrotoxic medications, and coordination with nephrology when needed all support kidney health.
For patients with obstruction of urine flow (which can occur from external compression by lymphomatous tissue), procedures to relieve obstruction—such as ureteral stent placement or, less commonly, nephrostomy tubes—may be needed. Obstruction often resolves with successful lymphoma treatment, though initial drainage may be necessary.
Response to treatment is typically assessed using PET/CT and clinical evaluation at intervals throughout and after therapy. Long-term follow-up is important because of the risk of recurrence, late effects of treatment, and, in some patients, the development of second cancers.
Supportive care, nutritional support, mental health support, and support for family and caregivers are important components of comprehensive care. Palliative care can be integrated at any point alongside cancer-directed treatment.
Care during pregnancy in patients with lymphoma requires special attention. Some treatments have implications for the fetus, and coordination with obstetrics, maternal-fetal medicine, and hematology-oncology is essential. Fertility considerations may be important for patients of reproductive age receiving treatment.
Care is best coordinated by multidisciplinary teams at centers with experience in lymphoma. Imaging, laboratory, pathology, and molecular findings are interpreted alongside the patient’s symptoms, examination, treatment history, and broader clinical context rather than in isolation.
Patient education plays an essential role. Understanding the diagnosis, the specific lymphoma subtype, the rationale for the recommended treatment plan, the meaning of imaging and laboratory findings during follow-up, treatment side effects, and warning signs of complications all contribute to better outcomes.
Red flag symptoms include severe flank or back pain, high fever with signs of severe infection (particularly during or shortly after chemotherapy), significant decrease in urine output, significant blood in the urine, severe abdominal pain, severe symptoms of tumor lysis syndrome (electrolyte disturbances, muscle cramping, weakness, decreased urination, or confusion), symptoms of severe electrolyte abnormalities, new severe back pain with weakness, numbness, or bowel or bladder changes (which may indicate spinal cord compression), difficulty breathing, or rapid clinical deterioration. These warrant prompt or urgent medical evaluation.
