Liver Lymphoma
Liver lymphoma refers to involvement of the liver by a cancer of the lymphatic system. It is far more often part of systemic lymphoma (secondary liver involvement) than a true primary liver lymphoma, which is rare. Liver lymphoma can present as one or more masses in the liver, diffuse infiltration of liver tissue, or enlargement of the organ without a discrete lesion. Most are non-Hodgkin lymphomas, with diffuse large B-cell lymphoma the most common subtype. Imaging—particularly CT, MRI, and PET/CT—and biopsy are central to diagnosis, and treatment depends on the lymphoma subtype and the patient’s overall situation.
What is it?
The lymphatic system is part of the body’s immune defense and includes lymph nodes, the spleen, bone marrow, the thymus, and lymphatic tissue scattered throughout many organs. Lymphoma is a cancer of the lymphatic system, in which abnormal lymphocytes (a type of white blood cell) grow and accumulate. Lymphoma can involve almost any organ, including the liver. Liver lymphoma describes involvement of the liver by a lymphoma, regardless of whether the disease started elsewhere and spread to the liver or—much less commonly—began in the liver itself.
Liver lymphoma can be broadly divided into two categories. Secondary liver involvement by lymphoma is far more common and occurs when systemic lymphoma spreads to the liver, often along with involvement of lymph nodes, the spleen, the bone marrow, or other organs. This is most often seen in advanced stages of non-Hodgkin lymphoma and can also occur in Hodgkin lymphoma. Primary hepatic lymphoma is rare and is defined by lymphoma that is confined to the liver, with no significant lymphoma elsewhere at the time of diagnosis. Primary hepatic lymphoma is most common in older adults and has been associated with chronic liver disease (particularly hepatitis B and hepatitis C), immunosuppression (including HIV and post-transplant patients), and autoimmune diseases. The most common subtype in both secondary and primary liver involvement is diffuse large B-cell lymphoma, although many other types and subtypes can affect the liver.
The patterns of liver involvement vary. Some patients develop one or more discrete liver masses or nodules, which can range in size from small to large. Others have diffuse infiltration of liver tissue without an obvious mass, sometimes producing only an enlarged liver and abnormal liver function tests. Some lymphomas can involve the bile ducts or the structures around the liver, producing additional features. In patients with established systemic lymphoma, liver involvement may be discovered during staging imaging or evaluation of liver function abnormalities.
Symptoms of liver lymphoma overlap with those of other liver diseases and lymphomas. Common features include upper right abdominal pain or fullness, fatigue, unintended weight loss, loss of appetite, nausea, and—particularly in lymphoma—systemic “B” symptoms such as unexplained fevers, drenching night sweats, and significant unintended weight loss. Enlargement of the liver (hepatomegaly) and spleen (splenomegaly) is common. Jaundice may occur, particularly when lymphoma infiltrates extensively, involves the bile ducts, or compresses biliary structures. Itching, fatigue, swelling, and easy bruising or bleeding can develop with significant liver dysfunction. Some patients present with vague symptoms that lead to imaging revealing liver lesions or enlargement.
Diagnosis combines clinical assessment, laboratory tests, imaging, and tissue confirmation. Blood tests typically include a complete blood count, comprehensive liver function tests, LDH (often elevated in lymphoma), uric acid, and viral hepatitis testing (particularly hepatitis B and C, both because of their association with certain lymphomas and because hepatitis B can reactivate during lymphoma treatment). HIV testing is often performed. Imaging usually begins with ultrasound, which may show liver enlargement, masses, or diffuse changes. CT with contrast and MRI with contrast provide detailed characterization of liver involvement and surrounding structures, as well as evaluation of lymph nodes and other organs. PET/CT is widely used for staging and for evaluating response to treatment in most lymphomas. Tissue diagnosis is generally required for definitive diagnosis and subtyping, often through image-guided core needle biopsy of a liver lesion or, in patients with accessible disease elsewhere, biopsy of another involved site. Specialized pathology with immunohistochemistry and molecular studies is essential to identify the specific lymphoma subtype, which strongly influences treatment.
Important to Know
Treatment of liver lymphoma is driven by the specific lymphoma subtype, the stage of disease, the patient’s overall health, and any associated conditions such as chronic viral hepatitis or immunosuppression. Care is typically coordinated by hematologists/oncologists with lymphoma expertise, working with hepatologists, infectious disease specialists, interventional radiologists, surgeons (in selected situations), and palliative care providers.
For most patients with diffuse large B-cell lymphoma involving the liver, chemoimmunotherapy regimens that include rituximab (an antibody targeting CD20) combined with chemotherapy form the standard backbone of treatment. Other B-cell lymphomas, T-cell lymphomas, and Hodgkin lymphomas have their own subtype-specific regimens. Targeted therapies, immune-based treatments (such as bispecific antibodies and chimeric antigen receptor [CAR] T-cell therapy in selected cases), and other novel agents are increasingly important options for certain lymphoma subtypes and in relapsed or refractory disease. Radiation therapy is used in selected situations.
Stem cell transplantation—autologous or allogeneic—plays a role in selected patients, typically in relapsed or specific high-risk lymphomas. Treatment plans are highly individualized.
Management of associated conditions is important. Patients with chronic hepatitis B or C require careful coordination with hepatology, both for direct treatment of the viral infection and to prevent hepatitis B reactivation during immunosuppressive therapy (often with prophylactic antiviral medications). Patients with immunosuppression, HIV, or transplant-related lymphomas require integrated care for their underlying condition.
Liver function may be affected both by the lymphoma itself and by treatment. Monitoring liver function tests, infection risk, and side effects is an essential part of care. Supportive measures—including nutrition, infection prevention with vaccinations and prophylactic medications when appropriate, transfusion support, and management of fatigue—play important roles.
Bile duct obstruction by lymphoma (particularly with large hilar disease or aggressive infiltration) may require endoscopic or percutaneous biliary drainage to relieve jaundice and support treatment. Tumor lysis syndrome—a metabolic complication that can occur when fast-growing lymphomas are treated—requires preventive measures and close monitoring at the start of therapy.
Supportive and palliative care play important roles throughout the course of disease. Pain management, nutritional support, mental health support, and family-centered care planning are essential. Early integration of palliative care, even alongside active treatment, is associated with improved quality of life.
Care is best provided in centers with experience in lymphoma and, where relevant, hepatology. Imaging and laboratory findings are interpreted alongside the patient’s symptoms, examination, and broader clinical context rather than in isolation.
Red flag symptoms include rapidly progressive jaundice; severe upper right abdominal pain; high fever with chills or signs of severe infection; severe weight loss with night sweats; signs of gastrointestinal bleeding; sudden confusion, severe lethargy, or difficulty arousing; severe leg swelling with shortness of breath; or signs of shock. These warrant prompt or urgent medical evaluation.
