Liver Fibrosis

Liver fibrosis is the buildup of scar tissue within the liver in response to chronic injury and inflammation. It develops gradually over years from many different underlying conditions, including fatty liver disease, alcohol-related liver disease, viral hepatitis, autoimmune liver disease, and other chronic conditions. Fibrosis is generally reversible in earlier stages, particularly when the underlying cause is treated, but it can progress to cirrhosis if injury continues. Modern noninvasive tools—including blood tests, ultrasound and MR elastography, and standard imaging—now play a central role in identifying and grading fibrosis, often without the need for biopsy.

What is it?

The liver responds to ongoing injury and inflammation by depositing fibrous (scar) tissue. This is a normal wound-healing process, but when it occurs continuously over many years it gradually distorts the liver’s architecture and impairs its function. Liver fibrosis refers to this buildup of scar tissue within the liver. Fibrosis exists on a continuum—from very mild (with little or no scarring) to advanced disease just short of cirrhosis (in which scarring becomes extensive and forms bridges throughout the liver), and ultimately to cirrhosis, in which scarring is so extensive that the liver’s structure is permanently altered.

Fibrosis is commonly graded using stages. The METAVIR system, originally developed for chronic hepatitis C, is widely used and ranges from F0 (no fibrosis) and F1 (minimal portal fibrosis) through F2 (some bridging fibrosis), F3 (numerous fibrotic bridges), and F4 (cirrhosis). Other staging systems exist for specific conditions, but the general principle is similar: stages reflect how much scarring has developed and how much the liver’s architecture has been affected. Advanced fibrosis (F3 or F4) is associated with significantly higher risks of complications, including portal hypertension, decompensated liver disease, and liver cancer.

Many different conditions can drive liver fibrosis. Metabolic dysfunction–associated steatotic liver disease (MASLD) and steatohepatitis (MASH)—formerly called nonalcoholic fatty liver disease and steatohepatitis—are now among the most common drivers and are tightly linked to obesity, diabetes, metabolic syndrome, and lipid abnormalities. Alcohol-related liver disease is another major contributor and often coexists with metabolic factors. Chronic viral hepatitis—particularly hepatitis B and C—remains an important cause globally, although effective antiviral therapies have dramatically improved outcomes. Autoimmune liver diseases include autoimmune hepatitis, primary biliary cholangitis (PBC), and primary sclerosing cholangitis (PSC). Genetic and metabolic causes include hemochromatosis, Wilson disease, and alpha-1 antitrypsin deficiency. Other contributors include chronic biliary obstruction, cardiac causes (such as long-standing right-sided heart failure or constrictive pericarditis producing congestive hepatopathy), certain medications and toxins, vascular conditions, and rare diseases.

A central insight in modern hepatology is that earlier stages of liver fibrosis are often reversible. When the underlying injury is controlled or removed, fibrosis can stabilize or significantly improve over time. This is particularly true in early fibrosis and is well documented in viral hepatitis (after viral suppression or cure), alcohol-related disease (after sustained abstinence), MASLD/MASH (after significant weight loss or metabolic improvement), and some autoimmune and metabolic conditions (with effective treatment). Advanced fibrosis and especially cirrhosis are less likely to fully reverse but can still stabilize and, in some cases, partially improve.

Many people with liver fibrosis—even at advanced stages—have no specific symptoms. Symptoms, when present, often reflect the underlying liver disease (such as fatigue or vague upper right abdominal discomfort) rather than the fibrosis itself. Symptoms of decompensated liver disease—such as ascites, variceal bleeding, hepatic encephalopathy, or jaundice—usually indicate cirrhosis or other advanced complications rather than fibrosis alone.

Diagnosis and staging of fibrosis have changed significantly in recent years and now rely heavily on noninvasive tools. Blood tests using validated scoring systems (such as FIB-4 and APRI, which use age, AST, ALT, and platelet count) are widely used as initial screening tools. More specialized blood panels (such as the Enhanced Liver Fibrosis or ELF test) can add information. Elastography—using ultrasound-based methods such as FibroScan or MR elastography—measures liver stiffness as a marker of fibrosis. These methods have largely replaced liver biopsy as the first-line tool for assessing fibrosis in many situations. Conventional imaging—ultrasound, CT, and MRI—provides additional information about liver size, contour, focal lesions, splenic enlargement, ascites, and signs of portal hypertension, particularly in advanced disease. Liver biopsy remains important in selected cases, particularly when the diagnosis is uncertain, when multiple causes overlap, or when staging is critical for management decisions.

Important to Know

The most important step in managing liver fibrosis is identifying and treating the underlying cause. Because fibrosis is potentially reversible in earlier stages, early identification offers a meaningful opportunity to change the trajectory of disease.

For MASLD/MASH, weight loss (typically 7–10% of body weight or more, when achievable), regular physical activity, a heart-healthy diet, control of diabetes and lipid abnormalities, treatment of obstructive sleep apnea, and limiting alcohol are foundational. In selected patients with steatohepatitis, newer disease-modifying medications are increasingly available. Bariatric surgery can be a powerful option in patients with severe obesity. For alcohol-related liver disease, abstinence from alcohol is the single most important intervention, along with support for alcohol use disorder, nutritional support, and management of complications. Chronic viral hepatitis is treated with antiviral medications, with hepatitis C now curable in most patients and hepatitis B managed long-term with effective antivirals. Autoimmune liver diseases are treated with immunosuppressive or specific medications under specialized care. Hemochromatosis is treated with phlebotomy, Wilson disease with chelation, and other metabolic and biliary conditions with disease-specific therapies.

Lifestyle measures support overall health and may help slow or even partially reverse fibrosis: nutritional optimization, regular physical activity, weight management, abstinence from alcohol, careful use of medications and supplements (since many are metabolized by the liver), and treatment of associated conditions such as diabetes, sleep apnea, and metabolic syndrome.

Surveillance is an important part of care. Patients with significant fibrosis or cirrhosis are typically followed with periodic imaging and laboratory testing to monitor disease progression, complications, and the development of hepatocellular carcinoma in selected populations. Repeated elastography or other noninvasive testing helps track changes over time. Patients with advanced fibrosis often benefit from screening for varices and other complications associated with portal hypertension.

Care is typically coordinated by primary care clinicians, hepatologists, gastroenterologists, dietitians, addiction specialists (when relevant), and other providers. Imaging and laboratory findings are interpreted alongside the patient’s symptoms, examination, and broader clinical context rather than in isolation.

Patients with liver fibrosis—including those with no current symptoms—benefit from clear education about the disease, modifiable risk factors, the importance of avoiding or limiting alcohol, vaccinations (such as for hepatitis A and B when not previously vaccinated), and the importance of regular follow-up.

Red flag symptoms include severe upper abdominal pain, persistent vomiting, vomiting blood or coffee-ground material, black or tarry stools, severe abdominal swelling, jaundice or dark urine, easy bruising or bleeding, confusion or sleep changes, severe lethargy, signs of infection (such as fever or chills), or sudden change in known liver symptoms. These warrant prompt or urgent medical evaluation, as they may indicate progression to cirrhosis or other serious complications.
