High Coronary Calcium Score

A high coronary calcium score is a result from a coronary artery calcium (CAC) scan—a low-dose CT scan of the heart—indicating that a significant amount of calcified plaque is present in the coronary arteries. The CAC score (often called the Agatston score) is one of the most powerful predictors of future cardiovascular events. A high score reflects more advanced atherosclerosis and a meaningfully higher risk of heart attack and other cardiovascular conditions. Identifying this finding early provides an opportunity for personalized, intensive preventive care.

What is it?

A coronary artery calcium (CAC) scan is a quick, low-dose CT scan of the heart that detects and measures calcium deposits within the walls of the coronary arteries. Calcium in these arteries is a marker of atherosclerosis—the gradual buildup of cholesterol-rich plaque that underlies most coronary artery disease. The amount of calcium identified is quantified as a CAC score, most commonly the Agatston score, which is calculated from the area and density of calcified plaque seen on imaging.

CAC scores are typically interpreted using broad categories. A score of 0 indicates no detectable calcified plaque and is associated with very low short-term risk of cardiovascular events. Scores of 1–99 indicate mild calcified plaque and modestly elevated risk; 100–299 indicate moderate calcified plaque and substantially higher risk; and 300 and above indicate extensive calcification and high risk. Many physicians consider scores of 100 or higher to be clinically meaningful, while scores of 300 or higher—and especially 1,000 or higher—typically prompt particularly intensive preventive care. CAC scores are also commonly interpreted as percentiles compared with people of the same age, sex, and ethnicity, providing context about whether a person’s plaque burden is higher than expected for their demographic group.

A high CAC score reflects more advanced atherosclerosis and a meaningfully higher risk of future cardiovascular events—particularly heart attacks—even in patients without symptoms. Importantly, CAC measures the total burden of calcified plaque rather than the severity of narrowing in any single artery; a person can have a high CAC score without having any severely narrowed arteries, though high scores are associated with a greater likelihood of significant stenosis. Soft (noncalcified) plaque is not directly measured by a CAC scan, but a high CAC score generally implies that noncalcified plaque is also present.

Many people with a high CAC score have no symptoms when the finding is identified. The CAC scan is most often performed as part of cardiovascular risk assessment in patients who are at intermediate risk or in whom the best preventive strategy is not clear. When symptoms are present—such as chest pressure or discomfort with exertion, shortness of breath, fatigue, or reduced exercise tolerance—they suggest possible flow-limiting coronary disease and typically prompt additional testing to assess for stenosis and ischemia.

After a high CAC score is identified, further evaluation depends on the patient’s symptoms, score, and overall risk. Patients without symptoms may proceed with intensified preventive care without additional cardiac imaging. Those with concerning symptoms, very high scores, or other features that raise concern may undergo additional testing such as coronary CT angiography (CCTA, which directly visualizes the coronary arteries and characterizes both calcified and noncalcified plaque), stress testing, or invasive coronary angiography when appropriate. The choice of next step is individualized.

A high coronary calcium score is best understood as a powerful early warning rather than an immediate emergency in patients without symptoms. It provides a personalized window into the actual extent of atherosclerosis—often years before symptoms develop—and an opportunity to take meaningful steps that can substantially reduce future risk of heart attack and other cardiovascular events.

Intensive risk factor management is the cornerstone of treatment. Statins are particularly important because they lower LDL cholesterol, stabilize plaque, and reduce the risk of cardiovascular events. In many patients with a high CAC score, statins are recommended even when overall risk by traditional calculators might appear borderline. Other lipid-lowering therapies (such as ezetimibe, PCSK9 inhibitors, and newer agents) may be added when more aggressive LDL lowering is needed. Blood pressure control, diabetes management (including agents with cardiovascular benefit such as SGLT2 inhibitors and GLP-1 receptor agonists when appropriate), and antiplatelet therapy in selected patients are tailored to individual risk.

Lifestyle measures are equally important and benefit nearly all patients: not smoking, regular physical activity, a heart-healthy diet (rich in vegetables, fruits, whole grains, healthy fats, and lean proteins; lower in saturated and trans fats, processed foods, added sugars, and excessive salt), weight management, limited alcohol, stress reduction, good sleep, and treatment of sleep apnea when present.

A high CAC score does not mean that every patient needs additional invasive testing or procedures. Many patients with high scores can be safely managed with intensified preventive therapy alone, particularly in the absence of symptoms. When chest pain, shortness of breath, or reduced exercise tolerance is present, additional testing—such as CCTA or stress testing—helps determine whether significant stenosis or ischemia is contributing.

Care is typically coordinated by primary care clinicians, cardiologists, and, in selected patients, preventive cardiologists or lipid specialists. Family members of patients with high CAC scores—particularly those with strong family histories of early heart disease—may also benefit from earlier risk assessment and preventive care.

Patients sometimes worry that a high CAC score means heart attack is inevitable. It does not. Modern preventive therapy has been shown to substantially reduce the risk of cardiovascular events, even in patients with significant plaque burden. The score is most useful as a guide for personalized treatment rather than a fixed prediction of future outcomes.

Red flag symptoms include sudden severe chest pain or pressure (especially radiating to the jaw, arm, or back), severe shortness of breath, fainting, palpitations with lightheadedness, signs of stroke, profuse sweating, or signs of shock. These warrant immediate emergency evaluation, as they may indicate a heart attack or another life-threatening cardiovascular condition.
