Hepatomegaly

Hepatomegaly is an enlargement of the liver beyond its normal size. It is a sign rather than a diagnosis in its own right and can result from many different conditions, including fatty liver disease, viral hepatitis, alcohol-related liver disease, heart failure, infections, infiltrative or storage disorders, cancers, and benign liver lesions. The clinical significance of hepatomegaly depends entirely on the underlying cause. Imaging—particularly ultrasound, CT, and MRI—plays a central role in measuring liver size and identifying contributing conditions.

What is it?

The liver is the largest internal organ in the body, located primarily in the upper right abdomen beneath the diaphragm. It plays a central role in many functions, including processing nutrients, producing bile, breaking down medications and toxins, regulating blood sugar, producing important proteins (such as clotting factors and albumin), and supporting immune function. Hepatomegaly is an enlargement of the liver beyond the size considered normal for the patient’s body habitus, age, and sex. The exact size threshold varies, but a liver length greater than approximately 15 to 16 centimeters in the midclavicular line, or imaging measurements above standard ranges, is often used to describe hepatomegaly. Hepatomegaly is a sign rather than a disease and is most useful as a starting point for identifying an underlying cause.

Many different conditions can lead to liver enlargement. One of the most common contemporary causes is steatotic liver disease (fatty liver), including metabolic dysfunction–associated steatotic liver disease (MASLD, formerly NAFLD) and steatohepatitis (MASH, formerly NASH), which are closely associated with obesity, diabetes, metabolic syndrome, and high cholesterol. Alcohol-related liver disease can also produce hepatomegaly, particularly in the early stages of disease. Viral hepatitis (hepatitis A, B, C, D, and E) can enlarge the liver during acute infection and, in chronic forms, contribute to long-term liver disease.

Cardiovascular causes include congestion of the liver from right-sided heart failure, constrictive pericarditis, severe tricuspid valve disease, and inferior vena cava obstruction. Infectious causes include bacterial, parasitic (such as amoebic, hydatid, and schistosomal infections in endemic regions), and other infections producing abscesses, granulomas, or generalized inflammation. Infiltrative and storage disorders—such as amyloidosis, sarcoidosis, hemochromatosis (iron overload), Wilson disease (copper accumulation), Gaucher disease, and other rare metabolic conditions—can produce significant enlargement. Cancers can affect the liver as primary tumors (such as hepatocellular carcinoma and intrahepatic cholangiocarcinoma) or, much more commonly, as metastases from cancers elsewhere in the body. Hematologic conditions such as leukemia, lymphoma, and certain myeloproliferative disorders can also produce hepatomegaly. Benign liver lesions—including large hemangiomas, focal nodular hyperplasia, and hepatic adenomas—can occasionally contribute. Autoimmune liver diseases (such as autoimmune hepatitis, primary biliary cholangitis, and primary sclerosing cholangitis) and biliary obstruction also need to be considered.

Symptoms vary widely. Many patients with mild hepatomegaly have no symptoms at all, and the finding is identified incidentally on imaging or physical examination. As enlargement progresses or as the underlying disease worsens, patients may experience vague upper right abdominal discomfort or fullness, early fullness with meals, fatigue, or weight changes. Specific symptoms often reflect the underlying cause and may include jaundice (yellowing of the skin and eyes), itching, dark urine, pale stools, leg swelling, abdominal swelling from ascites, easy bruising or bleeding, confusion or mental status changes (hepatic encephalopathy), gastrointestinal bleeding, or signs of heart failure when cardiovascular causes are present. Fever, night sweats, weight loss, and other systemic symptoms may suggest infection, infiltrative disease, or malignancy.

Evaluation begins with a careful history (including alcohol use, medications and supplements, exposures, family history, and risk factors for viral hepatitis), physical examination, and basic blood tests—including a complete blood count and liver function tests (such as ALT, AST, alkaline phosphatase, bilirubin, albumin, and prothrombin time/INR). Additional blood tests are tailored to the suspected cause and may include viral hepatitis panels, iron studies, ceruloplasmin (for Wilson disease), autoimmune liver disease antibodies, and tumor markers (such as alpha-fetoprotein). Ultrasound of the abdomen is often the first imaging study and is widely available, fast, and effective for assessing liver size, structure, masses, biliary system, and vascular flow. CT and MRI provide more detailed information and are particularly useful for characterizing lesions, evaluating diffuse liver disease, and assessing the surrounding structures. Elastography (using ultrasound or MRI) can assess liver stiffness as a marker of fibrosis. In selected cases, liver biopsy may be needed to establish a specific diagnosis.

Important to Know

Because hepatomegaly has so many possible causes, the most important step is identifying the underlying reason. Treatment depends entirely on what is contributing—and some causes are reversible or modifiable with targeted intervention.

For steatotic liver disease, weight loss, regular physical activity, a heart-healthy diet, control of diabetes and lipid abnormalities, treatment of obstructive sleep apnea, and limiting alcohol are foundational. Specific medications—including, in selected patients with steatohepatitis, newer agents that target the disease directly—are increasingly available and may be considered under specialized care. Bariatric surgery can be a powerful option for patients with severe obesity and significant metabolic disease. For alcohol-related liver disease, abstinence from alcohol is the single most important intervention, along with support for alcohol use disorder, nutritional support, and treatment of any complications.

Chronic viral hepatitis is treated with antiviral medications, with hepatitis C now curable in most patients with modern direct-acting antivirals, and hepatitis B managed long-term with effective antivirals. Vaccinations against hepatitis A and B reduce the risk of infection.

Other causes are managed based on the specific diagnosis: heart failure is treated with guideline-directed medical therapy; infections receive specific antimicrobial treatment; cancers are managed by oncology and hepatobiliary teams; infiltrative and storage disorders may benefit from disease-specific therapies (such as iron removal for hemochromatosis); autoimmune liver diseases may require immunosuppressive therapy; biliary obstruction may need endoscopic or surgical treatment; and benign liver lesions are typically monitored unless they cause symptoms or have concerning features.

Surveillance is important in patients with chronic liver disease, particularly those at risk for hepatocellular carcinoma (such as patients with cirrhosis or chronic hepatitis B). Regular imaging and laboratory follow-up help detect cancer early. Patients with advanced liver disease benefit from evaluation by hepatologists, and—when appropriate—consideration for liver transplantation.

Care is typically coordinated by primary care clinicians, hepatologists, gastroenterologists, infectious disease specialists, hematologists, oncologists, cardiologists, and other relevant providers. Imaging findings are interpreted alongside the patient’s symptoms, examination, laboratory results, and broader clinical context rather than in isolation.

Even when hepatomegaly is mild and asymptomatic, it is worth investigating because identifying the cause can lead to important interventions that prevent progression and improve overall health.

Red flag symptoms include severe upper right abdominal pain, persistent vomiting, jaundice, dark urine, pale stools, signs of gastrointestinal bleeding (such as blood in vomit or stool), significant abdominal swelling, severe fatigue with confusion, easy bruising or bleeding, severe leg swelling with shortness of breath, or sudden change in mental status. These warrant prompt or urgent medical evaluation.
