Gallbladder Cancer

Gallbladder cancer is an uncommon but aggressive cancer that arises from the cells lining the gallbladder. Most cases are adenocarcinomas, and most are associated with long-standing gallstones and chronic inflammation. Gallbladder cancer can develop silently and is often detected at an advanced stage; a substantial minority of cases are discovered incidentally during gallbladder surgery performed for stones or another reason. Outcomes are best when the cancer is detected early and completely removed. Imaging—particularly ultrasound, CT, MRI with MR cholangiopancreatography (MRCP), and endoscopic ultrasound—is central to diagnosis and staging.

What is it?

The gallbladder is a small, pear-shaped organ that lies beneath the right lobe of the liver. It stores and concentrates bile, which is then released into the small intestine after meals to help digest fats. Gallbladder cancer is a cancer that arises from the cells lining the gallbladder. The most common type is adenocarcinoma, which begins in the glandular cells of the gallbladder wall. Less common types include adenosquamous carcinoma, squamous cell carcinoma, neuroendocrine tumors, and rare sarcomas. Cancer can also spread to the gallbladder from other organs (metastatic disease), although this is uncommon.

Gallbladder cancer has a strong association with long-standing gallstones and chronic inflammation, which together create conditions that favor changes within the gallbladder wall over many years. Other recognized risk factors include older age, female sex, certain ethnic and geographic backgrounds, obesity, diabetes, and chronic infections such as those caused by Salmonella typhi and other organisms. Specific gallbladder conditions raise the risk: porcelain gallbladder (extensive calcification of the gallbladder wall), large gallstones (particularly greater than approximately 3 centimeters in some populations), large or atypical gallbladder polyps, primary sclerosing cholangitis, anomalous pancreaticobiliary duct junction, and Caroli disease with extensive biliary involvement, among others. Smoking, certain dietary patterns, and family history may also contribute.

Gallbladder cancer often develops silently because the gallbladder lies in a relatively protected location, and tumors can grow significantly before producing symptoms. Many cases are detected at an advanced stage, with cancer extending into the liver, surrounding bile ducts, lymph nodes, or distant organs. A notable subset of gallbladder cancers are discovered incidentally—after a gallbladder is removed for what was thought to be gallstone disease, the pathologist identifies cancer in the surgical specimen. This is one important reason that gallbladders containing high-risk features are usually examined carefully both before and after surgery.

Symptoms, when they occur, can include upper right abdominal pain or fullness, unintentional weight loss, loss of appetite, fatigue, nausea, vomiting, and a palpable mass in the right upper abdomen. Tumors that involve the bile duct or compress nearby biliary structures can cause jaundice, dark urine, pale stools, and itching. Advanced disease may produce ascites, leg swelling, or symptoms related to spread to other organs. The symptoms of early gallbladder cancer often overlap with those of common gallstone disease, which can delay diagnosis.

Diagnosis combines clinical assessment, blood tests, imaging, and—ultimately—pathology. Blood tests typically include liver function tests (often showing elevated bilirubin and alkaline phosphatase in patients with biliary obstruction) and tumor markers such as CA 19-9 and CEA, which may be elevated but are not specific. Ultrasound is often the first imaging test and may show wall thickening, a mass, or invasion of the liver. CT with contrast provides detailed information about local invasion, lymph node enlargement, and distant spread, and is widely used for staging and surgical planning. MRI with MR cholangiopancreatography (MRCP) is particularly valuable for evaluating biliary involvement and the liver, and for characterizing complex lesions. Endoscopic ultrasound provides high-resolution imaging and can guide fine-needle biopsy. Endoscopic retrograde cholangiopancreatography (ERCP) and percutaneous transhepatic cholangiography are used in selected patients for biliary drainage and sampling. Tissue confirmation is generally required before nonsurgical treatment and is typically obtained through biopsy of accessible tumor, cytology of bile or biliary brushings, or pathology of a resected specimen. Molecular testing is increasingly used in advanced disease to identify actionable targets such as HER2 amplification, FGFR2 fusions, IDH1 mutations, BRAF mutations, microsatellite instability, and others.

Important to Know

Treatment of gallbladder cancer is highly individualized and depends on the location and extent of the tumor, the stage at diagnosis, molecular features, and the patient’s overall health. Care is best delivered in centers with hepatobiliary cancer expertise and multidisciplinary teams that may include hepatobiliary surgeons, medical and radiation oncologists, hepatologists, interventional radiologists, gastroenterologists, pathologists, and palliative care specialists.

Surgical resection offers the best chance for long-term control or cure but is feasible only in a subset of patients. For very early disease confined to the gallbladder wall (such as a tumor invading only the inner layers), simple cholecystectomy may be sufficient. For more deeply invasive cancers, extended resection is typically required, including removal of part of the liver adjacent to the gallbladder bed, regional lymph nodes, and—in selected cases—portions of the bile duct or other surrounding structures, with reconstruction as needed. When cancer is found incidentally on pathology after a routine cholecystectomy, additional surgery is often considered to achieve clear margins and adequate lymph node assessment, with decisions individualized by stage, tumor location, and overall health.

Adjuvant chemotherapy after surgery is now used in many patients to reduce the risk of recurrence, often based on regimens used in biliary tract cancers more broadly. For advanced or unresectable disease, systemic therapy is the cornerstone of treatment. Standard first-line chemotherapy typically includes combinations such as gemcitabine plus cisplatin, often combined with immunotherapy in eligible patients. Targeted therapies are increasingly important: HER2-targeted treatments, FGFR2 inhibitors, IDH1 inhibitors, BRAF/MEK inhibitors, immunotherapy for microsatellite instability or high tumor mutational burden, and other agents are used based on molecular testing. Radiation therapy and locoregional treatments—such as transarterial chemoembolization or radioembolization—are options in selected patients.

Management of biliary obstruction is a critical part of care. Endoscopic or percutaneous biliary drainage with stent placement can relieve jaundice, reduce itching, prevent cholangitis, and support nutrition and treatment.

Supportive and palliative care play important roles throughout the course of disease. This includes pain management, nutritional support, treatment of itching, management of fatigue, support for psychological wellbeing, and family-centered care planning. Early integration of palliative care—even alongside active cancer treatment—is associated with improved quality of life.

Prevention and risk reduction focus on managing modifiable risk factors and timely treatment of high-risk gallbladder conditions. In some clinical situations, cholecystectomy is considered even in the absence of typical symptoms—for example, porcelain gallbladder, very large gallstones in selected populations, certain gallbladder polyps, and other specific scenarios. These decisions are individualized and based on guidelines and patient factors.

Surveillance after curative-intent surgery typically includes periodic imaging, blood tests, and clinical evaluation, with intervals individualized by stage, pathology findings, and overall risk. Patients with chronic conditions associated with gallbladder cancer risk—such as primary sclerosing cholangitis or Caroli disease—often undergo specific surveillance protocols.

Care is typically coordinated by hepatobiliary surgeons, medical oncologists, gastroenterologists, interventional radiologists, hepatologists, and primary care clinicians. Imaging and laboratory findings are interpreted alongside the patient’s symptoms, examination, and broader clinical context rather than in isolation.

Red flag symptoms include rapidly progressive jaundice; severe upper right abdominal pain; high fever with chills (which may suggest cholangitis); persistent vomiting; significant unintentional weight loss; signs of gastrointestinal bleeding; sudden confusion, severe lethargy, or difficulty arousing; severe abdominal swelling; or signs of sepsis or shock. These warrant prompt or urgent medical evaluation, as they may indicate cholangitis, biliary obstruction, or other serious complications.
