Dementia Evaluation

Dementia is a general term for significant, progressive decline in memory, thinking, and other cognitive abilities that interferes with everyday life. It is not a single disease; many different conditions can cause dementia. A dementia evaluation is a structured assessment that brings together history, examination, cognitive testing, blood work, and imaging—most often MRI of the brain—to identify the underlying cause, exclude treatable conditions, and guide care. Identifying the specific type of dementia is important because treatment, expected course, and support needs can vary substantially.

What is it?

Dementia is not a single disease but an umbrella term for a clinical syndrome characterized by significant, progressive decline in cognitive abilities—such as memory, language, attention, executive function, and visuospatial skills—that interferes with everyday life. A dementia evaluation is the structured process used to figure out whether dementia is present, how severe it is, what is most likely causing it, and how best to support the person and their family going forward.

Many different conditions can cause dementia. Alzheimer’s disease is the most common cause, accounting for the majority of cases in older adults. Vascular dementia, caused by reduced blood flow and strokes in the brain, is another major contributor and frequently coexists with Alzheimer’s (“mixed dementia”). Lewy body dementia and Parkinson’s disease–related dementia involve abnormal protein deposits and often present with visual hallucinations, fluctuating attention, parkinsonian motor symptoms, and prominent sleep disturbances. Frontotemporal dementias preferentially affect the frontal and temporal lobes and tend to present with behavioral changes or language problems rather than memory loss as the earliest feature. Less common causes include normal-pressure hydrocephalus, chronic traumatic encephalopathy, Creutzfeldt-Jakob disease, autoimmune and inflammatory encephalitis, infections, certain genetic disorders, and the long-term effects of alcohol and other substances. Some conditions that mimic dementia are actually reversible, such as severe depression, vitamin B12 deficiency, thyroid disease, medication side effects, and untreated sleep apnea.

A typical dementia evaluation begins with a careful conversation with the patient and—very importantly—with a family member, partner, or close friend who can describe changes over time. Topics include memory, language, problem-solving, judgment, behavior, mood, sleep, and the ability to handle daily activities such as managing finances, driving, cooking, and taking medications. A neurological examination assesses for signs that point toward specific causes (such as parkinsonian features, gait disturbance, or weakness). Structured cognitive testing—ranging from brief office-based tools to detailed neuropsychological evaluation—helps document the pattern and severity of cognitive change.

Laboratory tests are commonly used to look for reversible or contributing factors, including thyroid disease, vitamin deficiencies, infections, electrolyte abnormalities, and kidney or liver dysfunction. MRI of the brain is a central part of the evaluation because it can identify structural causes of cognitive impairment—including strokes, small-vessel disease, hydrocephalus, tumors, and chronic blood collections—and can show patterns of atrophy that support specific diagnoses (such as hippocampal atrophy in Alzheimer’s disease, asymmetric frontotemporal atrophy in frontotemporal dementias, or characteristic patterns in some other conditions). In specialized settings, additional studies such as FDG-PET (which shows patterns of brain metabolism), amyloid PET or tau PET (which detect Alzheimer-related proteins), cerebrospinal fluid biomarkers, and increasingly available blood-based biomarkers can refine the diagnosis.

Important to Know

A dementia evaluation has several important goals. The first is to determine whether the person’s symptoms truly represent dementia rather than normal aging, mild cognitive impairment, depression, or another condition. The second is to identify and treat any reversible or contributing factors, such as thyroid disease, vitamin deficiencies, sleep disorders, medication side effects, untreated hearing loss, or depression. The third is to determine the most likely specific cause, since this affects treatment options, expected course, and supportive needs.

Treatment is individualized. Some dementias have specific medications—such as cholinesterase inhibitors, memantine, and, in selected patients with early Alzheimer’s disease, newer anti-amyloid therapies—that may temporarily improve symptoms or, in some cases, modify the disease course. Management of cardiovascular risk factors (blood pressure, diabetes, cholesterol, smoking, weight, physical activity) is increasingly recognized as important for almost all types of dementia. Lifestyle measures—regular exercise, social engagement, cognitive activity, healthy diet, good sleep, hearing aids when needed—can also support brain health.

Equally important is supportive care: helping with safety in the home, planning around driving, organizing medications, supporting daily routines, addressing mood and sleep, and connecting patients and caregivers with community resources, support groups, and respite. Conversations about advance care planning, legal documents, and future preferences are best held early, while the person with dementia can fully participate.

Care is typically coordinated by primary care clinicians, neurologists, geriatricians, neuropsychologists, psychiatrists, social workers, and other supportive services. Caregivers play a central role and benefit greatly from education, support, and respite.

Red flag symptoms include sudden severe confusion or memory loss, sudden weakness or numbness, difficulty speaking, severe headache, falls, hallucinations, severe behavioral changes, or rapid worsening of cognition over days to weeks. These warrant urgent medical evaluation, as they may indicate stroke, infection, metabolic disturbance, or another condition that needs immediate attention.
