Chronic Pancreatitis
Chronic pancreatitis is a long-standing inflammatory disease of the pancreas in which repeated or ongoing injury leads to progressive damage of the pancreatic tissue, ductal changes, and gradual loss of function. The most common causes include long-term alcohol use, smoking, genetic conditions, autoimmune disease, and recurrent acute pancreatitis. Over time, chronic pancreatitis can produce persistent abdominal pain, poor digestion of fats and other nutrients, diabetes, and an increased risk of pancreatic cancer. Imaging—particularly CT, MRI with MR cholangiopancreatography (MRCP), and endoscopic ultrasound—is central to diagnosis and management.
What is it?
Chronic pancreatitis is a long-standing inflammatory disease of the pancreas, in which repeated or ongoing injury leads to progressive damage of the gland over months and years. The pancreas has two main jobs: producing digestive enzymes (which travel through the pancreatic ducts into the small intestine to help digest food) and producing hormones such as insulin and glucagon (which regulate blood sugar). In chronic pancreatitis, both functions are gradually impaired as the gland accumulates scarring (fibrosis), develops abnormal ductal patterns (often with stones and strictures), and loses functional tissue. The end result is a constellation of symptoms and complications that can include persistent pain, poor digestion, diabetes, and other consequences.
The causes of chronic pancreatitis are diverse. Long-term heavy alcohol use is one of the most common identified causes worldwide. Smoking is increasingly recognized as a major independent contributor and often interacts with alcohol use to accelerate disease. Recurrent acute pancreatitis—particularly when caused by alcohol, gallstones, or other ongoing factors—can evolve into chronic disease over time. Genetic conditions, including hereditary pancreatitis (often associated with mutations in PRSS1, SPINK1, CFTR, and CTRC), cystic fibrosis, and other inherited disorders, are well-recognized causes, particularly in younger patients or those with strong family histories. Autoimmune pancreatitis—including IgG4-related disease (type 1) and a separate non–IgG4-related form (type 2)—has distinct features and often responds well to specific treatment. Obstructive causes include strictures from prior injury, tumors, congenital anomalies such as pancreas divisum in selected patients, and persistent obstruction by stones. Other contributors include certain medications, radiation, severe hypertriglyceridemia, hyperparathyroidism, and—in some regions—tropical pancreatitis. Some cases have no identifiable cause and are labeled “idiopathic.”
The natural history of chronic pancreatitis is variable. Some patients experience predominantly painful episodes for years, while others develop more gradual, persistent symptoms or relatively pain-free progression toward exocrine and endocrine dysfunction. Pain is one of the most prominent and disabling symptoms in many patients. It often presents as upper abdominal pain radiating to the back, sometimes worsened by eating, and can be persistent or episodic. Pain mechanisms are complex and may involve neural, inflammatory, ductal, and central nervous system changes. As the disease progresses, exocrine pancreatic insufficiency develops in many patients, producing fatty or oily stools (steatorrhea), bloating, weight loss, fat-soluble vitamin deficiencies, and malnutrition. Endocrine insufficiency leads to diabetes (sometimes called type 3c or pancreatogenic diabetes), which is distinct from typical type 1 or type 2 diabetes and can be more challenging to manage. Less commonly, patients develop fluid collections (such as pseudocysts), narrowing of the bile duct or duodenum, splenic vein thrombosis with left-sided portal hypertension and gastric varices, or other complications. Chronic pancreatitis also increases the lifetime risk of pancreatic cancer.
Diagnosis combines clinical assessment, laboratory testing, and imaging, sometimes supplemented by tests of pancreatic function. Blood tests may include pancreatic enzymes (which can be normal in chronic disease even when symptoms are present), liver function tests, fasting glucose and HbA1c, complete blood count, and—in selected patients—IgG4 (for autoimmune pancreatitis), triglycerides, calcium, and genetic testing. CT of the abdomen with contrast can show characteristic findings including pancreatic atrophy, calcifications, ductal dilation, fluid collections, and complications. MRI with MR cholangiopancreatography (MRCP), often with secretin enhancement in select cases, provides detailed information about the pancreatic ducts, parenchyma, and biliary tree, and is particularly useful for identifying earlier disease. Endoscopic ultrasound (EUS) provides high-resolution imaging of the pancreas and can identify subtle changes, fluid collections, and lesions, and allows fine-needle biopsy when needed. ERCP is now used primarily for treatment rather than as a first diagnostic tool. Tests of exocrine function—such as fecal elastase—can document pancreatic insufficiency, while assessment for diabetes is increasingly recognized as essential.
Important to Know
Management of chronic pancreatitis is highly individualized and depends on the cause, severity of symptoms, presence of complications, and overall health. Care is typically delivered by multidisciplinary teams that may include gastroenterologists with pancreatic expertise, hepatobiliary or pancreatic surgeons, interventional radiologists, endocrinologists (for diabetes), dietitians, pain specialists, addiction medicine specialists, mental health providers, and primary care clinicians.
Lifestyle measures are foundational. Complete abstinence from alcohol is the single most important intervention for patients with alcohol-related disease and significantly improves outcomes. Smoking cessation is similarly critical, both because smoking accelerates disease progression and because it independently increases the risk of pancreatic cancer. A balanced, easily digestible diet—often with smaller, more frequent meals—supports nutrition. Maintaining a healthy weight and treating underlying metabolic conditions are important. In patients with severe hypertriglyceridemia, hyperparathyroidism, or other treatable contributors, specific medical management of these conditions is essential.
Pancreatic enzyme replacement therapy (PERT) is the cornerstone of treatment for exocrine pancreatic insufficiency. Taking pancreatic enzymes with meals and snacks helps the body properly digest fats, proteins, and carbohydrates; reduces steatorrhea and bloating; supports weight stability; and prevents fat-soluble vitamin (A, D, E, K) deficiencies. Supplementation of fat-soluble vitamins and bone health monitoring (with calcium, vitamin D, and bone density assessment as needed) are important parts of long-term care.
Management of pancreatogenic (type 3c) diabetes requires careful attention. Many patients require insulin, and blood sugars can be more variable than in typical type 2 diabetes. Coordination with endocrinology and individualized treatment plans help reduce the risk of hypoglycemia and other complications.
Pain management is complex and often requires multimodal strategies. Lifestyle changes, treatment of any reversible contributors, non-opioid medications, structured pain management programs, mental health support, and—when needed—careful and limited use of opioids may all play roles. Adjunctive treatments such as antioxidant therapy have shown variable benefit in trials. For selected patients with severe, refractory pain related to specific ductal abnormalities, endoscopic treatments (such as removal of pancreatic stones, sphincterotomy, and stent placement) can be effective. Extracorporeal shock wave lithotripsy (ESWL) is used to fragment larger stones. Surgical procedures—including drainage operations (such as the Puestow procedure), resection procedures (such as Whipple or distal pancreatectomy), or combined procedures (such as Frey or Beger operations)—are considered in selected patients with appropriate anatomy. Total pancreatectomy with islet auto-transplantation is offered in highly selected patients in specialized centers.
Management of complications is also essential. Pseudocysts may be observed if asymptomatic and stable, or drained endoscopically, percutaneously, or surgically when symptomatic, infected, or growing. Biliary or duodenal strictures from chronic inflammation may require endoscopic stent placement or surgical bypass. Splenic vein thrombosis with gastric varices may require specific management to prevent bleeding. Pseudoaneurysms within or adjacent to fluid collections may require interventional radiology treatment to prevent or treat bleeding.
Patients with chronic pancreatitis have an increased lifetime risk of pancreatic cancer—particularly with hereditary, smoking-related, and long-standing disease. Surveillance programs in specialized centers may be appropriate for selected high-risk patients. New or rapidly worsening pain, weight loss, jaundice, or other concerning symptoms warrant evaluation for malignancy.
Autoimmune pancreatitis is a distinct entity that often responds well to corticosteroids and, in selected patients, to immunomodulators or other targeted therapies. Distinguishing autoimmune pancreatitis from other forms is important because its treatment differs significantly.
Patient education and shared decision making are essential. Understanding the chronic nature of the disease, the importance of lifestyle changes (particularly alcohol and smoking), adherence to enzyme replacement and diabetes treatment, and recognition of warning signs all contribute to better outcomes.
Red flag symptoms include sudden severe abdominal pain, persistent vomiting, jaundice, dark urine, pale stools, high fever with chills (which may suggest cholangitis or infected fluid collections), significant unintentional weight loss, signs of gastrointestinal bleeding, sudden severe back pain (which may suggest a pseudoaneurysm or other vascular complication), severe diarrhea with dehydration, sudden confusion, low blood pressure, or signs of shock. These warrant prompt or urgent medical evaluation, as they may indicate complications such as acute pancreatitis, cholangitis, infected pseudocyst, bleeding, or—in some cases—pancreatic cancer.
