Cardiac Metastases
Cardiac metastases are deposits of cancer that have spread to the heart from a tumor that originated elsewhere in the body. They are much more common than primary heart tumors and can involve the heart muscle (myocardium), the lining around the heart (pericardium), the heart chambers, or the heart valves. Many cardiac metastases are discovered incidentally on imaging done for other reasons, while others present with symptoms such as chest discomfort, shortness of breath, arrhythmias, or fluid around the heart. Imaging—most often CT, cardiac MRI, and echocardiography—plays a central role in detection and characterization.
What is it?
Cardiac metastases are cancer deposits in the heart that have spread from a tumor originating elsewhere in the body. The heart can be involved in several different ways: the pericardium (the thin sac surrounding the heart) is the most common site, followed by the myocardium (the heart muscle), and, less commonly, the endocardium (the inner lining and chambers) and the heart valves. Cancer can also spread to the heart by extending along major veins—most notably the inferior vena cava—from tumors of the kidney, liver, or adrenal glands.
Although metastatic involvement of the heart is uncommon overall, it occurs much more frequently than primary heart tumors. Many cancers can metastasize to the heart, but the most common sources include lung cancer, breast cancer, melanoma (which has a particular tendency to involve the heart), lymphoma, leukemia, and certain gastrointestinal, genitourinary, and head and neck cancers. The exact pattern of cardiac involvement often depends on how the cancer spreads—through the bloodstream, the lymphatic system, direct extension from nearby structures, or growth along a major vein.
Symptoms vary widely and depend on the location, size, and number of metastases, as well as whether they affect the heart’s structure or function. Many cardiac metastases cause no symptoms and are discovered incidentally on imaging done for staging or follow-up of a known cancer. When symptoms occur, they may include chest discomfort, shortness of breath, palpitations, fatigue, leg swelling, signs of heart failure, or arrhythmias. Pericardial involvement can lead to fluid accumulation around the heart (pericardial effusion), which in severe cases may cause cardiac tamponade—a serious condition in which pressure on the heart prevents it from filling and pumping effectively. Tumor extending into a heart chamber can sometimes obstruct blood flow or cause embolic events.
Imaging is central to detecting and characterizing cardiac metastases. Echocardiography is often the first cardiac imaging test and can quickly identify masses, pericardial fluid, and effects on heart function. CT of the chest with intravenous contrast is widely used because it visualizes the heart, pericardium, lungs, mediastinum, and major vessels in detail and can show extension of tumor along veins or invasion from adjacent structures. Cardiac MRI provides excellent soft-tissue contrast and is particularly useful for characterizing the nature of a cardiac mass, distinguishing tumor from thrombus, and assessing the extent of involvement. PET/CT helps evaluate metabolic activity and look for additional sites of disease. In selected cases, fluid analysis from a pericardial drainage or biopsy of accessible tumor may be needed for diagnosis.
Important to Know
The discovery of cardiac metastases generally reflects advanced cancer, and management requires close coordination among multiple specialties, typically including oncology, cardiology, radiology, radiation oncology, and, when appropriate, cardiothoracic surgery. Treatment is directed both at the underlying cancer and at any cardiac complications.
Systemic treatment of the primary cancer—through chemotherapy, immunotherapy, targeted therapy, or hormonal therapy—is often the cornerstone of management, since cardiac metastases generally indicate widespread disease. Radiation therapy may be considered in selected cases, such as for symptomatic pericardial involvement. Pericardial effusions causing significant symptoms or cardiac tamponade may need to be drained urgently, sometimes followed by procedures such as pericardial window placement to prevent recurrence. Supportive cardiac care—including management of arrhythmias, heart failure symptoms, and blood thinning when appropriate—plays an important role. Surgery is occasionally used for specific situations, such as removal of tumor extending into the heart chambers along the inferior vena cava.
Because cardiac metastases can change the course and prognosis of cancer treatment, accurate diagnosis is important. Imaging findings are always interpreted alongside the patient’s overall clinical picture, including the type and stage of the primary cancer, the burden of disease elsewhere, and the patient’s symptoms and goals of care.
Red flag symptoms include severe shortness of breath, low blood pressure, fainting, severe chest pain, rapid or irregular heartbeat, marked leg swelling, or signs of cardiac tamponade. These warrant urgent medical evaluation.
