Bowel Lymphoma
Bowel lymphoma refers to lymphoma involving the small or large intestine. The gastrointestinal tract is the most common site of lymphoma outside the lymph nodes (extranodal lymphoma), and bowel lymphomas represent an important subgroup. Bowel lymphoma may occur as primary disease (arising within the bowel itself, without significant involvement elsewhere) or as secondary disease (part of widespread lymphoma affecting multiple sites). The most common subtypes include diffuse large B-cell lymphoma (DLBCL), mucosa-associated lymphoid tissue (MALT) lymphoma, mantle cell lymphoma, and, in specific settings, T-cell lymphomas. Symptoms often include abdominal pain, changes in bowel habits, weight loss, or gastrointestinal bleeding. Because bowel lymphoma typically responds well to lymphoma-directed treatment, distinguishing it from other bowel tumors is important, and biopsy is essential.
What is it?
Lymphoma is a cancer that arises from lymphocytes, the white blood cells that are central to the immune system. Lymphomas are broadly divided into Hodgkin lymphoma and non-Hodgkin lymphoma (NHL), and each includes many subtypes with distinct features and treatments. While lymphomas most commonly involve lymph nodes, bone marrow, and the spleen, they can also involve almost any other organ. The digestive tract is the most common site of extranodal (outside-of-lymph-node) lymphoma, and lymphomas can occur throughout the gastrointestinal system, from the esophagus to the rectum.
The stomach is the most common gastrointestinal site of lymphoma overall, followed by the small bowel. Colon and rectum lymphomas are less common. Within the bowel, the ileum is a particularly common site, likely because of its rich lymphoid tissue (including Peyer’s patches, which are aggregates of lymphoid tissue in the wall of the small bowel).
Bowel lymphoma can be classified in several ways.
By type, most bowel lymphomas are non-Hodgkin lymphomas of B-cell origin, though T-cell lymphomas occur and are important in specific settings. Common subtypes include:
– Diffuse large B-cell lymphoma (DLBCL) — the most common subtype overall, typically aggressive and requiring prompt treatment
– Mucosa-associated lymphoid tissue (MALT) lymphoma — a type of extranodal marginal zone lymphoma that most commonly affects the stomach (where it is often associated with H. pylori infection) but can also occur in the bowel; typically indolent
– Mantle cell lymphoma — an aggressive B-cell lymphoma that has a particular tendency to involve the bowel, sometimes producing a characteristic pattern called lymphomatous polyposis (multiple polypoid lesions throughout the bowel)
– Follicular lymphoma — an indolent B-cell lymphoma that can involve the bowel, often the small bowel
– Burkitt lymphoma — a highly aggressive B-cell lymphoma with a particular tendency to involve the abdomen (especially the ileocecal region) in children and young adults
– Enteropathy-associated T-cell lymphoma (EATL) — an aggressive T-cell lymphoma associated with celiac disease; requires prompt treatment
– Various other rarer subtypes
Hodgkin lymphoma involvement of the bowel is much less common than non-Hodgkin lymphoma involvement.
By whether the disease is primary or secondary:
– Primary bowel lymphoma refers to lymphoma that arises within the bowel wall and, at diagnosis, involves the bowel primarily without significant disease elsewhere
– Secondary bowel lymphoma refers to bowel involvement as part of widespread lymphoma that also affects lymph nodes, bone marrow, or other sites
Several factors increase the risk of bowel lymphoma. Immunosuppression—from HIV, medications following organ transplantation (post-transplant lymphoproliferative disorder, or PTLD), chemotherapy, or certain immunodeficiencies—significantly increases the risk. Celiac disease is a specific risk factor for enteropathy-associated T-cell lymphoma, and prolonged uncontrolled disease is thought to increase risk. Inflammatory bowel disease (Crohn’s disease and ulcerative colitis), particularly when treated with certain immunosuppressive therapies, is associated with a modestly increased risk. Helicobacter pylori infection is strongly associated with gastric MALT lymphoma; H. pylori’s role in bowel lymphomas is less established, though there is emerging evidence for associations between certain other infections and specific bowel lymphomas. Hereditary immunodeficiency syndromes (such as common variable immunodeficiency and severe combined immunodeficiency) increase risk.
Symptoms of bowel lymphoma are often nonspecific and can overlap with those of other bowel conditions. Common presenting symptoms include abdominal pain (often chronic and nonspecific), unintended weight loss, changes in bowel habits (diarrhea or, less commonly, constipation), gastrointestinal bleeding (which can be visible or detected only on stool testing, sometimes leading to iron-deficiency anemia), a palpable abdominal mass in some patients, and symptoms of malabsorption in some cases. Systemic (“B”) symptoms of lymphoma—unexplained fever, drenching night sweats, and unintended weight loss—may be present.
In some cases, bowel lymphoma presents acutely with complications such as bowel obstruction (from tumor mass or luminal narrowing), bowel perforation (particularly with more aggressive lymphomas such as DLBCL or during rapid response to treatment), significant bleeding, or, less commonly, intussusception (telescoping of one segment of bowel into another, more common in specific pediatric presentations of Burkitt lymphoma).
Diagnosis of bowel lymphoma is based on imaging, endoscopy, and biopsy.
CT of the chest, abdomen, and pelvis is a primary imaging test and often shows characteristic findings, including bowel wall thickening (which can be quite pronounced in lymphoma, sometimes more than in other bowel cancers), mass formation, associated lymphadenopathy (particularly in the mesentery and retroperitoneum), and, in some cases, a specific pattern in which the bowel wall is thickened but the lumen remains open (aneurysmal dilation) — an appearance that can be characteristic of lymphoma. PET/CT is central for lymphoma staging, treatment response assessment, and follow-up.
Endoscopy provides direct visualization and biopsy for accessible sites. Upper GI endoscopy evaluates the stomach and duodenum. Colonoscopy evaluates the colon, terminal ileum, and rectum. For the mid and distal small bowel—which is difficult to reach with standard endoscopy—capsule endoscopy or deep enteroscopy (with balloon-assisted techniques) may be used. When these are not feasible or in specific circumstances, image-guided or surgical biopsy may be needed.
Biopsy is essential for diagnosis. Because lymphoma diagnosis and subtyping require adequate tissue for immunohistochemistry, flow cytometry, and molecular studies, larger biopsies (such as core needle biopsy or surgical biopsy) are often preferred over small biopsies (such as fine-needle aspiration alone).
Staging follows standard lymphoma principles and typically includes PET/CT, bone marrow evaluation in selected cases, blood tests (complete blood count, lactate dehydrogenase [LDH], comprehensive metabolic panel), and, when clinically indicated, evaluation of the central nervous system. Additional evaluation may include testing for viral infections (such as HIV, Epstein-Barr virus, and hepatitis B and C) that are relevant to lymphoma risk and treatment planning, and evaluation of cardiac function before receiving cardiotoxic chemotherapy.
For patients with suspected specific subtypes, additional evaluation may include tests such as small bowel biopsy for celiac disease evaluation (particularly when EATL is suspected), H. pylori testing, and appropriate viral studies.
Important to Know
Management of bowel lymphoma follows lymphoma-directed treatment principles based on the specific subtype, stage, patient factors, and, when relevant, molecular characteristics. Care is best coordinated by multidisciplinary teams that include hematology-oncology specialists, gastroenterologists, surgeons, pathologists (particularly hematopathologists), radiologists, nutrition specialists, and, when needed, other specialists. Importantly, unlike most other bowel cancers, surgery is generally not the primary treatment for bowel lymphoma—rather, systemic therapy is central.
For diffuse large B-cell lymphoma of the bowel—the most common aggressive lymphoma involving the bowel—standard treatment typically includes rituximab plus a combination chemotherapy regimen such as R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisone). Duration and specific approach depend on stage and other factors. Central nervous system prophylaxis with intrathecal chemotherapy or systemic high-dose methotrexate is considered in patients with specific risk factors, including extensive extranodal involvement.
For MALT lymphoma of the bowel, treatment principles are similar to gastric MALT lymphoma in some respects. For localized indolent disease, treatment options may include radiation therapy alone, rituximab, chemoimmunotherapy, or, for asymptomatic patients, observation in specific settings. When H. pylori is identified in gastric MALT lymphoma, antibiotic therapy alone can lead to complete regression in a substantial proportion of patients, though the role of antibiotic therapy in bowel MALT lymphoma is less established.
For mantle cell lymphoma involving the bowel, treatment typically follows standard mantle cell lymphoma principles, which have evolved substantially with the availability of BTK inhibitors (such as ibrutinib, acalabrutinib, and zanubrutinib), lenalidomide, and other targeted therapies, in addition to chemoimmunotherapy regimens. Stem cell transplantation may be considered in appropriate patients.
For follicular lymphoma involving the bowel, treatment depends on the stage, symptoms, tumor burden, and patient factors. Options range from observation (in some asymptomatic patients) to rituximab-based therapy alone or chemoimmunotherapy.
For Burkitt lymphoma, treatment is highly intensive and involves specific chemotherapy regimens (such as CODOX-M/IVAC, DA-EPOCH-R, or others) delivered at centers experienced in this aggressive lymphoma. Central nervous system prophylaxis is essential.
For enteropathy-associated T-cell lymphoma (EATL), treatment is often intensive and outcomes have historically been poorer than for many other lymphomas. Treatment typically includes multi-agent chemotherapy, and, in appropriate patients, autologous stem cell transplantation. Strict adherence to a gluten-free diet is important for the underlying celiac disease. Care at experienced centers is beneficial.
For patients with post-transplant lymphoproliferative disorder (PTLD) involving the bowel, treatment often begins with reduction of immunosuppression in transplant recipients, along with rituximab in many cases and, when needed, chemotherapy. Coordination with transplant medicine is essential.
Surgery is generally reserved for specific circumstances in bowel lymphoma rather than as primary treatment. Indications for surgery may include: bowel perforation (either at presentation or during treatment), bowel obstruction that cannot be managed medically, significant bleeding that cannot be controlled with other measures, and, in selected specific circumstances such as localized primary bowel lymphoma of certain subtypes, resection with curative intent. Bowel perforation during chemotherapy is a particular concern in aggressive lymphomas that respond rapidly to treatment, as the rapidly dying tumor tissue can weaken the bowel wall.
Care of nutritional status is important, particularly for patients with pre-existing weight loss, malabsorption, or bowel involvement that affects intake. Consultation with nutrition specialists can be helpful.
Tumor lysis syndrome—a rapid breakdown of tumor cells that can release large amounts of electrolytes and cause acute kidney injury—is a particular risk in patients with high tumor burden and rapidly proliferating lymphomas (such as Burkitt lymphoma and some cases of DLBCL). Preventive measures with hydration, medications to reduce uric acid (such as allopurinol or rasburicase), and careful monitoring of electrolytes and kidney function are essential.
Care during pregnancy in patients with bowel lymphoma requires special attention. Some treatments have implications for the fetus, and coordination with obstetrics, maternal-fetal medicine, and hematology-oncology is essential. Fertility considerations may be important for patients of reproductive age receiving treatment.
Response to treatment is typically assessed using PET/CT and clinical evaluation at intervals throughout and after therapy. Long-term follow-up is important because of the risk of recurrence, late effects of treatment, and, in some patients, the development of second cancers.
Supportive care, mental health support, and support for family and caregivers are important components of comprehensive care. Palliative care can be integrated at any point alongside cancer-directed treatment.
Care is best coordinated by multidisciplinary teams at centers with experience in lymphoma. Imaging, laboratory, pathology, and molecular findings are interpreted alongside the patient’s symptoms, examination, past medical history, and broader clinical context rather than in isolation.
Patient education plays an essential role. Understanding the diagnosis, the specific lymphoma subtype, the rationale for the recommended treatment plan, the meaning of imaging and laboratory findings during follow-up, treatment side effects, and warning signs of complications (including bowel perforation, obstruction, or tumor lysis syndrome) all contribute to better outcomes.
Red flag symptoms include severe abdominal pain, sudden severe abdominal pain (which may suggest perforation), symptoms of bowel obstruction (severe abdominal pain, distention, vomiting, inability to pass stool or gas), significant gastrointestinal bleeding, high fever with signs of severe infection (particularly during or shortly after chemotherapy), severe symptoms of tumor lysis syndrome (electrolyte disturbances, muscle cramping, weakness, decreased urination, or confusion), significant weight loss with dehydration, new severe back pain with weakness, numbness, or bowel or bladder changes (which may indicate spinal cord compression), or rapid clinical deterioration. These warrant prompt or urgent medical evaluation.
