Atrial Enlargement

Atrial enlargement is an increase in the size of one or both of the heart’s upper chambers (atria) beyond what is expected. It is more often a sign of an underlying condition than a disease in its own right. Common causes include high blood pressure, valve disease, atrial fibrillation, heart failure, certain cardiomyopathies, and chronic lung disease. Atrial enlargement is associated with an increased risk of arrhythmias—particularly atrial fibrillation—and other cardiovascular complications. Echocardiography, cardiac MRI, and ECG are the central tools for identifying and assessing it.

What is it?

The heart has four chambers: the right atrium and left atrium (the upper chambers, which receive blood) and the right ventricle and left ventricle (the lower chambers, which pump blood). The atria receive blood returning to the heart and pass it on to the ventricles. Atrial enlargement is an increase in the size of one (or both) of these upper chambers beyond what is expected for the patient’s body size, age, and sex. Atrial enlargement is more accurately a sign of an underlying problem than a disease in itself, and identifying the cause is generally more important than the finding alone.

The atria typically enlarge in response to increased pressure or volume that they must handle over time. Left atrial enlargement is most commonly associated with conditions that elevate pressure or volume on the left side of the heart, including longstanding high blood pressure, left ventricular hypertrophy or stiffness (diastolic dysfunction), mitral valve disease (particularly mitral regurgitation or stenosis), and atrial fibrillation. Right atrial enlargement is more commonly associated with conditions that elevate pressure or volume on the right side of the heart, such as chronic lung disease (including COPD and pulmonary hypertension), tricuspid valve disease, pulmonary valve disease, certain congenital heart anomalies, and right-sided heart failure. Both atria can be enlarged when there is widespread heart disease, such as severe cardiomyopathy, advanced heart failure, or long-standing atrial fibrillation.

A particular feature of atrial enlargement is its close relationship with atrial fibrillation. Atrial enlargement increases the risk of developing atrial fibrillation, and atrial fibrillation in turn promotes further atrial enlargement—a self-reinforcing cycle. Atrial fibrillation and atrial enlargement together significantly increase the risk of stroke, heart failure, and other complications, and treatment often considers both findings together.

Symptoms vary depending on the underlying cause and the broader effects on the heart. Mild atrial enlargement may cause no symptoms at all and is often discovered incidentally on imaging. As enlargement progresses or as the underlying condition worsens, symptoms may include palpitations (often from atrial fibrillation or other arrhythmias), shortness of breath (especially with exertion or when lying flat), fatigue, reduced exercise tolerance, lightheadedness, leg swelling, abdominal swelling, or signs of heart failure. In some cases, symptoms reflect the underlying condition more than the atrial enlargement itself.

Evaluation typically begins with ECG, which may suggest atrial enlargement through changes in the P wave, although ECG findings are not specific or sensitive enough for definitive diagnosis. Echocardiography is the primary imaging test for measuring atrial size and assessing chamber function, valve function, ventricular size and pumping function, and pressures. Cardiac MRI provides highly accurate three-dimensional measurements of atrial volumes and can offer additional information about tissue characteristics, scarring (such as atrial fibrosis), and the function of other heart structures. Chest X-ray and CT may also show signs of atrial enlargement, particularly when significant. Ambulatory rhythm monitoring helps identify atrial fibrillation or other arrhythmias. Blood biomarkers (such as BNP or NT-proBNP) may provide information about associated heart strain or failure.

Important to Know

Management of atrial enlargement is directed at identifying and treating the underlying cause and reducing the risk of associated complications. Specific interventions depend on the contributing conditions.

For atrial enlargement related to high blood pressure or left ventricular stiffness, aggressive control of blood pressure with medications and lifestyle changes is central. Preferred medication classes are tailored to individual factors and often address both blood pressure and additional cardiovascular goals. For atrial enlargement related to mitral valve disease, treatment may involve monitoring, medications, or—when valve disease is severe and meets criteria—repair or replacement. For right atrial enlargement related to pulmonary hypertension or chronic lung disease, treatment is directed at those underlying conditions.

Atrial fibrillation is a particularly important consideration in patients with atrial enlargement. Management may include rate control or rhythm control strategies, anticoagulation to reduce stroke risk (based on individual risk scores), and, in selected patients, catheter ablation or other procedures. Early treatment of atrial fibrillation in patients with significant atrial enlargement may help reduce further chamber remodeling.

Heart failure related to atrial enlargement is managed with guideline-directed medical therapy tailored to whether ejection fraction is preserved or reduced. Lifestyle measures—including a heart-healthy diet, regular physical activity (individualized), weight management, treatment of sleep apnea, limited alcohol, stress management, and good control of blood pressure, cholesterol, and diabetes—support overall cardiovascular health.

Some forms of atrial enlargement related to congenital heart conditions or specific cardiomyopathies require specialized care, often coordinated with congenital heart disease specialists, electrophysiologists, and cardiothoracic surgeons.

Care is typically coordinated by primary care clinicians and cardiologists, with input from electrophysiologists, heart failure or valve specialists, pulmonologists, and other relevant specialists. Imaging findings are interpreted alongside symptoms, examination, and other test results.

Atrial enlargement is generally not “reversed” in a complete sense, but its underlying drivers can often be slowed or improved. Even modest improvements in blood pressure control, weight, sleep apnea treatment, and arrhythmia management can have meaningful effects on cardiovascular outcomes.

Red flag symptoms include severe or worsening shortness of breath, severe chest pain, fainting, sustained rapid or irregular heart rate, severe leg swelling, sudden weight gain, signs of stroke (such as sudden weakness, numbness, or difficulty speaking), or signs of shock. These warrant prompt or urgent medical evaluation.
