Asplenia / Absent Spleen

Asplenia is the absence of a functioning spleen. It can be congenital (present from birth), surgical (after splenectomy), or functional (the spleen is present but no longer working properly, such as in sickle cell disease). Because the spleen plays important roles in filtering blood and defending against certain infections, patients without a functioning spleen are at increased lifetime risk of serious infections—particularly from encapsulated bacteria—and benefit from a clear plan of vaccinations, preventive measures, and prompt evaluation of fevers. Imaging can confirm the absence or change in the spleen and help identify causes of functional asplenia.

What is it?

The spleen is an organ in the upper left abdomen that plays several important roles, including filtering aging or abnormal blood cells from the bloodstream, supporting the immune system, helping to recognize and respond to certain infections (particularly those caused by encapsulated bacteria), and—in some circumstances—producing blood cells. Asplenia is the absence of a functioning spleen, which can occur through several distinct mechanisms.

Congenital asplenia is the rare condition of being born without a spleen. It may occur as an isolated finding (which is very uncommon) or, more often, as part of a broader congenital syndrome such as heterotaxy syndromes (in which the position and development of multiple organs is affected). Patients with congenital asplenia often have other anomalies, including complex congenital heart disease and abnormalities of abdominal organs, and require lifelong coordinated care.

Surgical asplenia—asplenia after splenectomy—is the most common form. Splenectomy may be performed for many reasons, including trauma, hematologic conditions (such as immune thrombocytopenia refractory to medical therapy, hereditary spherocytosis, certain lymphomas and leukemias, and selected myeloproliferative neoplasms), splenic abscess or rupture, certain splenic tumors or cysts, splenic torsion (such as in wandering spleen), and others. Splenectomy can be total (the entire spleen removed) or partial (a portion of the spleen left in place, which can preserve some splenic function).

Functional asplenia is a condition in which the spleen is present but no longer functions normally. The most common cause is sickle cell disease, in which repeated splenic infarctions during childhood lead to gradual loss of splenic function (“autosplenectomy”). Other causes include severe celiac disease, certain autoimmune conditions, advanced cirrhosis, infiltrative or storage disorders, prior radiation, and rare other conditions. Patients with functional asplenia carry many of the same infection risks as those with surgical asplenia.

The most important clinical consequence of asplenia is increased lifetime risk of certain serious infections. Without a functioning spleen, the body is less able to clear specific types of bacteria from the bloodstream, particularly encapsulated organisms such as Streptococcus pneumoniae (pneumococcus), Neisseria meningitidis (meningococcus), and Haemophilus influenzae type b. Patients without a functioning spleen are also at increased risk of severe infections from certain other pathogens, including some animal bite–related bacteria (such as Capnocytophaga canimorsus) and parasites such as Babesia and malaria in endemic regions. The most feared complication is overwhelming post-splenectomy infection (OPSI), a rapidly progressive sepsis syndrome with very high mortality if not treated promptly. Although the absolute risk of OPSI per year is relatively low, the consequences are severe enough that preventive measures and rapid response to fevers are essential.

Other potential consequences of asplenia include changes in the appearance of blood cells (such as the presence of Howell-Jolly bodies and other findings on peripheral blood smear), modestly increased platelet counts after splenectomy, and—in selected patients—an increased risk of certain blood clots (thrombosis), particularly in conditions such as hereditary spherocytosis after splenectomy. Long-term outcomes vary by underlying cause and overall health.

Diagnosis of asplenia is typically straightforward based on history (such as prior splenectomy) and imaging. Ultrasound, CT, or MRI can confirm the absence or marked atrophy of the spleen. In patients with suspected congenital asplenia or functional asplenia from sickle cell disease or other conditions, peripheral blood smear may show characteristic findings such as Howell-Jolly bodies (small DNA remnants normally cleared by the spleen). In selected patients, scintigraphic studies can assess splenic function and identify accessory splenic tissue, which may sometimes contribute residual function.

Important to Know

Management of asplenia centers on three pillars: vaccinations, prompt evaluation and treatment of suspected infections, and patient education. With careful adherence to these measures, the great majority of patients live healthy lives with low rates of serious complications.

Vaccinations are the foundation of infection prevention. Recommended vaccines for patients without a functioning spleen typically include pneumococcal vaccines (often involving both conjugate and polysaccharide formulations), meningococcal vaccines (including conjugate vaccines against multiple serogroups and vaccines against group B), and Haemophilus influenzae type b vaccine when not previously received. Influenza vaccination annually, COVID-19 vaccination per current recommendations, and other vaccines (such as zoster vaccines in age-appropriate adults) are also important. Travel-related vaccines may be added based on destinations. The specific schedule depends on age, vaccination history, and current guidelines, and is best coordinated through primary care, infectious disease, or—in pediatric patients—pediatric specialists.

Antibiotic prophylaxis is recommended in selected patients, particularly in children for a period after splenectomy and in patients considered to be at especially high risk. Adult use of long-term prophylactic antibiotics is more individualized and depends on age, time since splenectomy, prior infections, and other factors. In addition, many patients are encouraged to have a supply of “standby” antibiotics for use during a suspected serious infection while seeking medical care.

Prompt evaluation of fevers is essential. Any patient with asplenia who develops fever or signs of significant infection should seek medical evaluation urgently, even if symptoms seem mild. Healthcare providers should be informed of asplenia so that empiric treatment can be initiated rapidly when appropriate. Patients are encouraged to wear a medical alert tag or carry documentation of their asplenia.

Specific risks deserve special attention. Animal bites—particularly dog bites—warrant prompt evaluation and antibiotic treatment because of the risk of Capnocytophaga and other infections. Tick bites and travel to areas where Babesia or malaria are present warrant specific precautions. Travel to regions with high rates of meningococcal, pneumococcal, or other infections may require additional vaccinations or prophylaxis.

Underlying conditions are managed in coordination with the relevant specialists. Sickle cell disease, congenital syndromes, autoimmune conditions, cirrhosis, and other contributors require their own ongoing care. Patients with congenital syndromes (such as heterotaxy) may need cardiology, surgical, and other specialty involvement throughout life.

Care is typically coordinated by primary care clinicians and, when relevant, infectious disease specialists, hematologists, cardiologists, surgeons, and other providers. Patients and families benefit from clear written guidance, including a personalized plan for vaccinations, what to do for fevers, when to use standby antibiotics, and how to seek emergency care.

Travel guidance is an important component of long-term care. Patients without a functioning spleen who plan to travel internationally—particularly to areas with malaria, certain bacterial infections, or limited medical resources—benefit from pre-travel consultation, vaccinations, prophylactic medications, and detailed plans for managing illness while away.

Red flag symptoms in patients with asplenia include any fever (particularly high fever), chills, rapid heart rate, low blood pressure, severe headache, neck stiffness, confusion, severe weakness, signs of sepsis, severe abdominal pain, or rapid clinical deterioration. These warrant immediate emergency evaluation, as they may indicate overwhelming post-splenectomy infection or other serious infections.
