Cleerly explained: What AI sees in your arteries that a standard heart scan misses

A coronary CT shows your arteries. Cleerly’s AI measures what’s actually inside them — plaque volume, plaque type, and ischemic risk, vessel by vessel.

Most heart imaging answers one main question: are your arteries narrowed? It’s a useful question, but it isn’t the whole story. Plenty of heart attacks happen in arteries that don’t look severely narrowed on a scan — because the real danger isn’t always how wide the artery is. It’s what’s inside the wall.

Curious what AI can find in your arteries?

What a coronary CTA shows

A coronary CT angiogram (CCTA) uses IV contrast and a high-resolution scanner to produce detailed images of your coronary arteries. A radiologist reads those images visually — identifying narrowings, mapping calcified plaque, and flagging anything that looks unusual. This already puts a CCTA well ahead of a basic calcium score, which only measures calcium and returns a single number. But “looks moderate” or “appears mild” is still a human estimate of a three-dimensional structure on a two-dimensional screen.

What Cleerly adds

Cleerly is an AI analysis platform that runs on the same CCTA data your radiologist is already reading. Instead of estimating, it quantifies — measuring total plaque volume, separating calcified plaque from non-calcified (soft) plaque, evaluating stenosis severity in both 2D and 3D, and flagging ischemic risk indicators across every coronary segment. The output is a structured, reproducible report: not “moderate disease in the LAD,” but specific numbers for how much, what kind, and where — the kind of detail that’s hard to eyeball, even for an expert.

The Bottom Line

A standard CCTA shows where there’s plaque. Cleerly tells you how much, what kind, and what it likely means for your long-term risk.

Who it’s for

Cleerly is built for people who want more than a yes-or-no answer about heart disease. That includes anyone with a family history of cardiac events, anyone whose prior testing came back inconclusive, anyone managing risk factors over time, and anyone interested in catching disease before it becomes symptomatic. At CPAI it’s offered as a CCTA with Cleerly analysis ($1,550) or with added ischemia analysis ($1,850), performed on an open-ring scanner with reports delivered within 48 hours. Most appointments are available within a week.
